Life Event: Dependent Moves To or From the USA

This Life Event occurs when your dependent loses their health insurance as a result of their having a change in residence either from outside of the USA to within the USA, or from within the USA to outside of the USA.

Changes You Can Make and How To Make Them

When this Life Event happens, you can make any of the below changes to your benefits, provided eligibility requirements are met. Expand a menu for details.


  • The below changes can only involve the dependent who moved to/from the USA; you cannot make changes to your other dependents’ coverage as part of this Life Event.


If the dependent has moved to the USA within the past 60 days, you can enroll them in your medical and/or dental coverage.

To make this change:

  1. Gather/scan documentation that serves as proof of your dependent’s move:
    • Documentation that serves as proof of the dependent’s relationship to you – refer to the Dependent Verification page for details.
      -AND-
    • Proof of the dependent’s loss of coverage, i.e. a letter or document (such as a certificate of creditable coverage) showing that their coverage was lost within the past 60 days.
      -AND-
    • Either:
      • The dependent’s visa or passport stamped with a date of entry to the US that occurred within the past 60 days.
        -OR-
      • Proof of the dependent’s former and current residence, such as a utility bill or bank statement dated within the past 60 days.
  2. Contact UWHR Benefits to let us know your dependent has moved to the USA and needs to be enrolled on your current medical and/or dental plan.
  3. Once instructed by UWHR Benefits to do so, complete your enrollment change in Workday according to the procedure in the Dependent Moves To/From the USA User Guide.

Effective Date
Your enrollment will take effect on the 1st of the month after the later of either:

  1. The date dependent changes residence from other country to the United States,
    -OR-
  2. The date you submit your requested enrollment changes in Workday.

If the later of a. or b. is the 1st of the month, the enrollment date is that day.

If the dependent has moved from the USA within the past 60 days, you can remove them from your medical and/or dental coverage.

To make this change:

  1. Gather/scan documentation that serves as proof of your dependent’s move – provide one of the following:
    • The dependent’s visa or passport stamped with a date of entry that occurred within the past 60 days.
      -OR-
    • Proof of the dependent’s former and current residence, such as a utility bill or bank statement dated within the past 60 days.
  2. Contact UWHR Benefits to let us know your dependent has moved from the USA and needs to be removed from your current medical and/or dental plan.
  3. Once instructed by UWHR Benefits to do so, complete your enrollment change in Workday according to the procedure in the Dependent Moves To/From the USA User Guide.

Effective Date
Removal of your dependent from your coverage will take effect on the last day of the month of the later of either:

  1. The date your dependent changes their residence from the US to another country,
    -OR-
  2. The date you submit your requested enrollment changes in Workday.

If the later of a. or b. is the 1st of the month, the remove date is the last day of the previous month.

If you have previously waived (declined) medical coverage, you can enroll yourself in medical coverage in order to enroll a dependent who, within the past 60 days, has moved to the USA.

To make this change:

  1. Download and complete the Employee Enrollment Change form linked in the right hand sidebar of this page.
  2. Gather/scan the following supporting documentation:
    • Documentation that serves as proof of the dependent’s relationship to you – refer to the Dependent Verification page for details.
      -AND-
    • Documentation that serves as proof of your dependent’s move:
      • The dependent’s visa or passport stamped with a date of entry that occurred within the past 60 days.
        -OR-
      • Proof of the dependent’s former and current residence, such as a utility bill or bank statement dated within the past 60 days.
        -OR-
      • Proof of the dependent’s loss of coverage, i.e. a letter or document (such as a certificate of creditable coverage) showing that their coverage was lost within the past 60 days.
  3. Mail or email (preferred) the completed form and supporting documentation to UWHR Benefits.

Effective Date
Your enrollment will take effect on the 1st of the month after the later of either:

  1. The date dependent changes residence from other country to the United States,
    -OR-
  2. The date the Employee Enrollment Change form is received by UWHR Benefits.

If the later of a. or b. is the 1st of the month, the enrollment date is that day.
