This life event occurs when you acquire a new dependent(s) due to a birth or adoption, or when you assume legal obligation for full or partial support prior to an adoption.
As part of this life event, you can make any of the below changes – expand a menu for instructions on how to make that particular change.
Changes To Your Dependents
Add newborn or newly adopted child(ren) to your medical/dental plan(s)
If enrolling your newborn/newly adopted child within 60 days of the birth/finalized adoption:
- Gather the required dependent verification documents.
- Follow the Birth or Adoption User Guide for step-by-step instructions on submitting your changes in Workday. Have the documentation you prepared in step 1 handy, as you’ll need to upload it to Workday.
IMPORTANT
If it is beyond 60 days of the birth/finalized adoption, instead of the above steps, please contact UWHR Benefits.
Effective Date
Enrollment of your newborn or newly-adopted child is effective on the day of birth, adoption, or assuming legal obligation.
Add spouse or State Registered Domestic Partner (SRDP) to your medical/dental plan(s)
You may add your spouse or SRDP to your medical and/or dental plan(s) within 60 days of the birth/finalized adoption only if you are also adding a newborn or newly-adopted child as a dependent.
To do so:
- Download and complete the Employee Enrollment Change form linked in the right-hand sidebar of this page.
- Mail or email (preferred) the completed form to UWHR Benefits.
You cannot add your spouse or SRDP to your medical and/or dental plan(s) after 60 days of the birth/finalized adoption.
Effective Date
The effective date for the coverage is the 1st of the month in which the child was born or adopted.
Remove dependent(s) from your medical/dental plan(s)
You may remove dependents from your medical and/or dental plan within 60 days of the birth/finalized adoption only if the dependent being removed enrolls in other group coverage due to the birth or adoption of a child. Follow the instructions for submitting a Gain of Other Coverage Life Event.
You cannot remove a dependent from your medical and/or dental plan after 60 days of the birth/finalized adoption.
Effective Date
The effective date will be whichever of the following dates is later:
- the last day of the month in which the child was born or adopted
-or-
- the last day of the month in which your form is received by UWHR Benefits.
Changes To Your Medical/Dental Plan(s)
Enroll in or change medical/dental plan(s) for you and your dependents
Within 60 days of the birth/finalized adoption you can:
- Change your medical and/or dental plan if you enroll a new child in your coverage.
-or-
- Return from waived enrollment in medical coverage to enroll your child.
To do so:
- Download and complete Employee Enrollment Change form linked in the right-hand sidebar of this page.
- Mail or email the completed form to UWHR Benefits.
You cannot change your medical and/or dental plan after 60 days of the birth/finalized adoption.

The effective date for your coverage is the 1st of the month in which the child was born or adopted.
Waive (decline) your medical plan
If waiving your medical coverage within 60 days of the birth/finalized adoption, follow the instructions for submitting a Gain of Other Coverage Life Event.
You cannot waive your medical coverage after 60 days of the birth/finalized adoption.
Effective Date
When you waive your medical coverage as part of a birth or adoption Life Event, the effective date will be whichever of the following dates is later:
- the last day of the month in which the child was born or adopted
-or-
- the last day of the month in which your form is received by UWHR Benefits.
You cannot waive your PEBB dental coverage, which is provided at no cost to you; however, you can waive dental coverage for your dependents.
Changes to Other Benefits
Enroll in an FSA or the DCAP/Change your FSA/DCAP contributions
Within 60 days of the birth/finalized adoption:
- Prepare your supporting documentation – scan documents that serve as proof of the event, i.e. a copy of birth/hospital certificate or placement letter from adoption agency.
- Download and complete the Change In Status form linked in the right-hand sidebar of this page.
- Mail or email the completed form and supporting documentation to UWHR Benefits.
You cannot enroll in FSA/DCAP or change your FSA/DCAP contributions after 60 days of the birth/finalized adoption unless/until you have another qualifying life event.
