Life Event: Change In Residence

This Life Event is when you or your dependent have a change in residence which affects the availability of your/their current medical and/or dental plans: if the current plan does not provide coverage in the new location, a different medical and/or dental plan can be chosen.
Changes can be made no later than 60 days after the date of your change in residence.
Important Points
- If you are moving, and your current medical and dental plans are still available to you in your new location, you do not qualify for this Life Event.
- A dental plan is considered to be available if a provider is located within 50 miles of your new residence.
- The only changes that can be made as part of this Life Event are choosing a new medical and/or dental plan. (In other words, you cannot make other changes such as waiving your coverage, adding new dependents to your coverage, or removing existing dependents from your coverage.)
- If you miss the 60-day deadline, you and any currently enrolled dependents will be automatically enrolled (“defaulted”) into the Uniform Medical Plan (UMP) Classic plan. Your next opportunity to change your coverage would be Open Enrollment in November or if you experience a qualifying Life Event.
How To Make Changes
To change your selected medical and/or dental plan, follow these steps no later than 60 days after the date of your change in residence:
- If you haven’t already done so, depending on whose address has changed, update your/your dependent’s address in Workday to reflect your/their new address.
- Contact UWHR Benefits to let us know you have moved and are no longer in the service area for your current medical and/or dental plan.
- Gather/scan documentation that serves as proof of your former AND current residences, such as a utility bill, bank statement, or US Postal Service change of address confirmation letter that has your name on it.
The document you provide must be dated within the last 60 days. - Follow the steps in the Life Event – Change in Residence User Guide to submit your changes in Workday. As part of that procedure, you’ll upload (“attach”) the documentation from the previous step to Workday.
Effective Date
Your changes take effect on the 1st of the month after the later of either:
- The date of your change in residence
-OR- - The date you submitted your changes in Workday.
If the later of a. or b. is the 1st of the month, the effective date is that day.
